Intermittent P0174-00 Code on '13 5.0L V8 w/ 130k Miles

I know somewhere in this vast body of threads this has been discussed already but I have searched every different combination of words I can come up and I am still coming up short. I was getting a code for both bank 1 and bank 2 running lean a few weeks ago and had an accompanying rough idle. It was pretty clear that my PCV valves were torn and causing the issue so I ordered a fresh pair and popped them on. While I was in there I also put in a new MAP sensor (I had a P0106 code for "Manifold Absolute Pressure/Barometric Sensor Range/Performance" but it most likely was due to the PCV valves as well), new air filters, and cleaned my MAF sensors. This fixed the idle issue and solved the lean code for bank 1 but I am still getting a lean code for bank 2. I smoke tested the car pretty thoroughly while it was warm and can't find any other vacuum leaks but was hoping someone more knowledgeable than I am may have some solutions.

Are there any common leaks I am missing that would not be revealed from a smoke test?

Is it possible just the MAF sensor on bank 2 is bad? I feel like since the system blends the air from both sides of the intake this would create a lean on both banks not just one. I also don't have the GAP tool but have a Foxwell scanner and the readings/voltages for both MAF sensors look consistent/no large anomaly.

You could try swapping MAF sensors and see if the code goes away. If it does then you need sensors on both channels replaced.

I have the same codes I am trying to track down for my LR3. I've noticed that the clamp holding the air box hose on keeps shifting and coming apart no matter how tight I tighten the clamp. This seems to let more air in and may be causing my code.cleared this morning and made it to work without the code coming back

@stone4567 Not likely a MAF if it’s affecting just one bank. Vacuum leaks also tend to affect both banks. If you can do it with your tool, check lambda and fuel trims to make sure the lean is just on one bank. If not, maybe try replacing the upstream o2 sensor on the bank with the code. Lean on one bank is often o2 sensor. Could also be injector, but I think o2 is more common (and easier to swap).

It's comments like this that make me feel infinitely embarrassed. I switched them around and will wait for the code to come back. It hasn't had any codes since last week so who knows. I will follow up when the code comes back.

I have the foxwell and it is not the best at pulling historic data unfortunately. I tried watching real time but everything looks within normal ranges. The code is extremely intermittent. I can go 100 miles without any issues then it will set the CEL light off out of nowhere then go back to normal the next time I crank the car.
