I'm posting this in case anyone else had a similar issue.
While the iPhone4 is connected and playing music from it, answering a phone call and then disconnecting does not automatically go back to playing music. Display reports that the device is paused. Nothing done from the display rectifies this.
Up until now, disconnecting and reconnecting the iPhone has resolved the issue.
Today: upgrading the iPhone OS to version 4.3.5 (8L1) has resolved the issue.
